Our Mission
We strive to build a community of inclusion for young adults throughout the Fox Valley area (i.e., Appleton, Kaukauna, Oshkosh, Green Bay, etc.) looking for purpose, friendship and continued learning of critical life skills as they progress from high school to adulthood. We define ourselves as a facility based adult day service. Being a facility based adult day service provider we strive to offer consultative and therapeutic services such as coaching, training in the area of daily living skills and recreation all while making connections in the community. As educators, co-founders Bryanna Moder and Shannon Diedrick saw a need in the greater Fox Valley area to bring additional facility based adult day services to support individuals who benefit from a community atmosphere. Inclusion's mission focuses on developing programming uniquely designed for all members as we strive to ensure individual growth based on unique needs.

What We Offer
Designed with unique needs in mind!
The ICC is a community based adult day service provider.
Inclusion Community Center LLC provides services for young adults with intellectual and physical disabilities. Daily services include life skill training, structured recreation, community engagement opportunities, supported engagement, and coaching with counseling consulting. Co-founders Bryanna and Shannon started Inclusion after working with students of varying ages and abilities during their careers as educators in the Fox Cities.
Life Skills Training and Structured Recreation
Inclusion Community Center LLC is unique in our approach as we work closely with each client to learn specific needs to provide the right level of services and programming. By focusing on what members need, our approach and model has the potential to shift and change to accommodate the needs of clients within our community. Our strengths in collaboration allow us to build connections to additional community services that benefit our client base.

As stated by Wisconsin DHS (Department of Health Services), adult program services in training and recreation are defined as the following;

"Training services provide a planned and systematic sequence of formal and informal activities for adults designed to develop skills in performing activities of daily and community living including self-help, motor and communication skills and to enhance emotional, personal and social development."

"Recreation services are activities designed to meet specific individual needs such as individual self-expression, social interaction and entertainment; develop skills and interests leading to enjoyable and constructive use of leisure time; and improved well-being."

Inclusion Community Center LLC works to increase engagement for our clients by combining training services and recreation. As each clients plan and goals are unique to them, the focus in these areas are not always a perfect balance as we are committed to ebb and flow with clients needs as they arise knowing that both are necessary to attain set goals.
Coaching and Consulting
Coaching and consulting services benefit individuals and family family units to implement routines and strategies within their specific environment.
Defined by Wisconsin DHS, counseling services as; "Services that provide professional guidance based on knowledge of human behavior through the use of interpersonal skills to achieve specific goals. Construction and implement routines and strategies within their specific environment to enhance positive life skills, recreation and leisure activities."

In the home environment, family coaching, routine and environment structure are just a few services that can be provided. Inclusion Community Center LLC staff want to support individuals and families to understand the members capabilities and how to strengthen physical and interpersonal skills.
